| YOUR VISITPLEASE KEEP US UPDATEDWhen you come in, please advise the receptionist of any change in your address, phone, marital status, or insurance coverage that may have occurred since your last visit. You will be asked to present your insurance card at each visit.
FINANCIAL POLICIESThere is a fee for our services. We participate in and file claims with the following:
Co-Pays are due at time of service. Statements are sent out monthly and indicate both the patient and insurance due balances. AS A COURTESY, we file claims with other insurance plans as well, but if we have no response after 60 days, we will bill you directly. YOUR HEALTH INSURANCE IS A CONTRACT BETWEEN YOU AND YOUR INSURANCE COMPANY, AND IT IS YOUR RESPONSIBILITY TO PURSUE UNPAID CLAIMS PROMPTLY. Our office relies on you to settle your account, regardless of your medical insurance coverage. For your convenience, we accept MasterCard, Visa, personal checks, and cash as a means of payment for your account balance. Monthly payment plans are available and can be arranged by contacting our billing office. Our staff will be happy to answer your questions regarding insurance, account balances, and payment plans. To speed your visit along, print the two forms from these links, fill them out, and bring them to the receptionist at our offices.
PRESCRIPTION RENEWALSPrescriptions and refills are issued during office hours. To have your prescriptions refilled, please have your pharmacist call our pharmacy line, (319) 358 - 2733, during regular business hours. |
